Our office does not accept any HMO policies such as BCN (Blue Care Network nor do we accept any Medicaid policies. 

Due to time constraints in the office, we kindly ask that you complete the new patient paperwork AHEAD of your scheduled appointment time. The paperwork takes about 15-30 minutes to complete.  If you are unable to complete the paperwork ahead of time, please contact our office for alternative arraignments prior to your appointment.  If you are unable to complete the paperwork prior to your appointment, we will need to reschedule your appointment to a later date.  We appreciate your understanding.

• 3) Complete the intake form that is emailed or texted to you at the time of scheduling  

There are two sets of forms to complete.  The first set of forms will be either texted or emailed to you when you call to setup your initial appointment. Please completed these within 24 hours as the access code will expire after 24 hours.

The second set of forms is a pdf file.  I recommend saving the pdf file to your computer. Please print them off and bring them with you on your first visit. 

Completing these forms ahead of your scheduled appointment will save you significant time in the office and allow us to provide you with the best possible care. On your first visit to our office, you can expect a warm welcome and a thorough consultation. We will review your paperwork and take the time to discuss your medical history, current health concerns, and any specific pain or discomfort you're experiencing. We will perform a chiropractic examination, which could include assessing your posture, range of motion, and reflexes. Based on this evaluation, we may or may not need to take X-rays.  We will develop a personalized treatment plan tailored to your needs.  If you are a candidate for chiropractic care, your initial visit will likely include gentle treatment with the goal of alleviating discomfort, promoting overall well-being and starting your journey toward better health.
