New Patient Center

Due to time constraints in the office, we kindly ask that you complete the new patient paperwork AHEAD of your scheduled appointment time. The paperwork takes about 30 minutes to complete.  If you are unable to complete the paperwork ahead of time, please contact our office for alternative arraignments prior to your appointment.  If you are unable to complete the paperwork prior to your appointment, we will need to reschedule your appointment to a later date.  We appreciate your understanding.

You are just 3 easy steps to having an appointment.

• 1) Complete My Chiro Forms
• 2) Complete PDF
• 3) Call or Email for an appointment time.

There are two sets of forms to complete.  The first set is called "My Chiro Forms".  When filling out the "My Chiro Forms" please only use a computer with a mouse. Do not use a tablet or phone. To fill out these forms, click on the "My Chiro Forms" link below. At the bottom, there is a spot to sign, place your mouse in the box, click and hold down the left mouse button, and sign (I recommend doing this FIRST before filling out the rest of the form. Patients have complained that they filled the form out and then had trouble signing it.) Your signature will look like a 3-year-old signed. When completed hit send, there is no printing needed. These will be directly submitted to our office.

The second set of forms is a pdf file.  I recommend saving the pdf file to your computer. You can either fill them out and print them off and bring them with you on your first visit or you can fill them out and email them over using the email address listed below.  

Once you have completed the pdf forms (save them), please email the file to info@wellness4lifechiro.com.

Completing these forms ahead of your scheduled appointment will save you significant time in the office and allow us to provide you with the best possible care. Our office does not accept any HMO policies such as BCN (Blue Care Network nor do we accept any Medicaid policies.
